3/15/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Ms. Broadnax reported that Anthony continues exhibit at-risk behaviors within the home and community. Ms. Broadnax reported that Anthony continues to make threats towards her and others. 2.Behavior: Ms. Broadnax reported that Anthony approached her with an aggressive threatened tone when she questioned him about coming right home off the bus. Ms. Broadnax reported that Anthony started utilizing profane language and slamming doors because he was upset with his mother. 1.Intervention: Upon arrival for todays session, QMHP-C continued to assessed and monitored Anthony’s behaviors towards authority figures and others. QMHP-C discussed with Anthony about his interactions are inappropriate and he needs to show respect to authority figures and others. QMHP-C informed Anthony that he cannot become aggressive and abusive when he feels things are not going his way. QMHP-C encouraged Anthony when he interacts his peers to communicate instead of being forceful and aggressive. 2.Intervention: QMHP-C allowed Anthony to ask questions along with brainstorming ideas during today’s session to assist him with understanding the effects of his behaviors within the home and community. QMHP-C discussed positive ways to assist Anthony with understanding how to follow directions without exhibiting reported behaviors within the home. QMHP-C modeled skills through role play for Anthony. QMHP-C encouraged along with Anthony to take notes during today’s session. 1.Response: Anthony went on to report that he got upset with his mother when she started to questioned him more about not coming home right after he gets off the bus as he verbalized he wanted to play with his friends. Anthony had to be redirected several times during session. Anthony became upset as his mother continue to talk about his behaviors within the home, community and school and his safety as well as her concern as a parent. 2.Response: Anthony reported that he will try to reframe from making threats when he is upset with his mother and cannot have his way. Anthony stated that he will call QMHP-C if he feels like hurting someone . Anthony stated that he will utilize what he learned during todays session. Anthony was a upset when QMHP-C asked him about his current school and behaviors as he got in trouble with his TDT counselor.
